Data and Survey Requests

 

Please review the UNT Health Quick Facts and the UAA website for official institutional data. If you don’t see the data that you need, submit a ServiceNow request for a consultation, data or reports, survey design or a customized data solution.

Initial Data Requests

To submit a data request to the Planning and Decision Support team, click the Data Request Form button. If you would like to request additional data for a previous data request, or expand the scope of a currently submitted data request. Please use the Data Request Resubmission Form.

Note: University Assessment & Accreditation's Planning and Decision Support team operates on a standard minimum turnaround time of two weeks. Emergency and System requests are given the highest priority and a standard turnaround time of one week.

Supplemental Data Requests

To submit an additional request related to a previous or current data request, click the Data Request Resubmission Form button.

Survey Requests

Note: Delivery times will vary based upon the complexity of the survey request, prioritization of institutional projects and individual requests.
